After completing a physician-led program, many patients choose to continue their care with Uplift For Her through our Annual Membership. To ensure we can continue providing personalized care, timely access, and ongoing support, our physicians intentionally limit the size of their patient panels. This membership model allows established patients to maintain a close relationship with their team while preserving the high level of service, communication, and accessibility our clinic is known for.
Annual Membership:
$325 per year

- Ongoing physician-patient relationship and continuity of care
- Portal communication related to active treatment plans, prescription management, lab review, and follow-up questions
- Prescription management and refill support
- Personally curated lab orders based on your individual needs

Follow-up Care:
Functional medicine follow-up appointments are available on a fee-for-service basis for established patients and for patients who have recently completed a physician-led program and wish to continue their care.
